We kindly urge everyone to prepare early and follow all instructions carefully.

KEY INSTRUCTIONS – MUST BE COMPLETED BY AUGUST 25, 2025

PASSPORT APPLICATION:

1. Complete the application and make payment at: www.immigration.go.tz
2. Email your form and documents to:  ppappt@tanzaniaembassy-us.org

• Birth certificate
• Parent’s citizenship certificate or sworn affidavit
• Proof of residence
• Old passport (if applicable)

 NIDA (National ID):

1. Download the application form from: www.nida.go.tz
2. Fill it out in capital letters using a black pen
3. Email your form and documents to:  fmnida@tanzaniaembassy-us.org

• Valid passport
• Birth certificate
• Parent’s documents (certificates or affidavits)
• Proof of residence
